As an Independent Facilitator, my role is to provide you with the personalized support and guidance you need to take control of your services and achieve your goals. What is Self Determination?
California Self Determination Program (SDP) provides individuals and their families more freedom to choose services that fit their unique needs and goals.
You have the opportunity to choose your own services and service providers to better meet your needs.
My Services
At Rad Advocacy, I am dedicated to fostering Self-Determination and enriching the lives of individuals with developmental disabilities through a comprehensive Person-Centered Plan.
What to expect: A collaborative approach that places the individual at the forefront of the decision-making, ensuring their unique strengths, goals, and preferences are the foundation of their support services. The Person Centered Plan will be a guided conversation between the individual and whoever they choose to join to create a holistic plan.
This meeting can be arranged as one long session or broken up into several mini sessions to be able to accommodate everyone’s needs.
This service provides continuous, hands-on assistance for individuals and families who are actively enrolled in the Self-Determination Program (SDP). The goal is to ensure you can successfully manage your services, budget, and goals with confidence and ease, fully realizing the freedom and flexibility of the program.
What to expect: Regular check-ins to evaluate what is working, identify new challenges or unmet needs, and brainstorm solutions. As goals change, you will get assistance with modifying your spending plan to reflect what is most important to you. You will receive support in locating services, processing invoices, and budget tracking.
This ongoing support service empowers you to handle the day-to-day details of self-determination, so you can focus on living the life you have chosen. This service will be provided as needed through the SDP budget year. The level of support is individualized based on your needs.
